Our series on the death of 7-month-old Lily Adair (Wade) revealed that a Child Protective Services investigator had for months been given evidence that the baby was being neglected and failed to step in. The investigation forced the head of the Texas Department of Family and Protective Services to admit that employees of his agency failed to respond properly to the warning signs. Lily's mother was eventually charged in her daughter's death.
